Atención Padres de Familia: 

Para que sus hijos (menores de 15 años) puedan recibir un obsequio, usted debe completar el formulario accediendo a través del codigo QR que aparece en el flyer, o tambien visitando nuestras oficinas. La fecha de registro termina el 15 de Diciembre del 2026. 

Se dará prioridad a los niños que estén registrados. Sin embargo, existe la posibilidad de que los niños  puedan recibir un obsequio si la familia no está registrada. Los niños deben estar presentes en el evento  para recibirlo.  

Favor anotar el nombre de su hijo(a), edad, fecha de nacimiento y género. Tenga en cuenta que es solo para menores de 15 años. Si necesita anotar más niños debe rellenar esta misma aplicación por cada niño/(a) que desee recibir obsequios.
